Disability Assessment
It is an electronic service, which enables people with disabilities who are currently registered in the ministry as beneficiaries to submit a request to updating their disability data, or the people with disabilities who are not registrar yet in the ministry to submit a request to register their disability data and registering them as beneficiaries of ministry to be able to benefits the services provided to people with disabilities.

- Choose the disability assessment service from the list of services on the electronic portal.
- Fill in the required data (social and guardian data), attach medical reports, approve the declaration and pledge, then submit the application.
- The application number and details will be sent via text message.
- After submitting the disability assessment application, the status of the application can be followed up from the "My Requests" icon option
- Log in to the portal and select "Disability Evolution" services from electronic services.
- Fill out Disability Evolution's form and upload your medical report then submit the request.
- The request number and detail will be sent via text messag
- The status of a Disability Evolution can be tracked from option "My Requests".
- Log in to the portal., You should have a profile on the portal.
- Attach the original medical report issued by a hospital accredited by the Ministry.
- Its validity period must not exceed one year from the date of issuance.
- Fill out the required data and agree to the declaration and pledge.
